The game's storyline follows the events of the "Brand New Reality" montage sequence from the Disney Channel Original movie, where Phineas, Ferb, Candace, Agent P and Dr. Doofenshmirtz jump into various dimensions while being pursued by evil Normbots. The game fills in some gaps on what happened during this chaotic dimension-hopping. Players will also encounter references to certain episodes of the TV show that were not featured in the movie.

The gameplay is a typical platformer with puzzles, mini-games, and boss battles, as well as exploring and fighting basic enemies. Players can co-op play with a friend or team up with a computer-controlled partner. Collecting items and unlocking special characters will be crucial to progress, as well as customizing weapons with different colors and sound effects.

The game offers many achievements/trophies to win, an increased difficulty mode that becomes available after achieving the final trophy, and background props that change depending on the time of year being played.
