The story of Double Dragon takes place in a post-apocalyptic New York, where twin brothers Billy and Jimmy Lee run a small martial arts school. Their lives take a drastic turn when Billy's girlfriend, Marian, is kidnapped by the "Black Warriors", a savage street gang led by Willy. The Black Warriors demand the Lee brothers disclose their martial arts secrets in exchange for Marian's freedom, prompting them to embark on a rescue mission.

The gameplay follows the Lee brothers as they navigate through the city slum, industrial area, and forest to reach the Black Warriors' hideout. Using various techniques such as punches, kicks, and weapon usage, the brothers must fight their way through hordes of enemies to confront Willy in a final showdown.

The NES version of Double Dragon also features an additional VS. fighting mode for one or two players. In this mode, players can choose from six characters, including Billy and five enemy characters, and engage in a one-on-one fighting match until only one player's health bar is depleted.
