Wolfenstein: The Old Blood is a standalone expansion to 2014's Wolfenstein: The New Order . Players step into the shoes of Polish-American OSA Operative William "B.J." Blazkowicz, this time in a prequel to the events that kick off the original games' plot. The year is 1946 and the Allies are losing the war due to the Nazis' technological superiority. General Wilhelm Strasse, known as "Deathshead", is developing technology that could cripple the Allies. To defeat him, the Allies must find his hidden compound. However, only one other member of the Reich knows its location: Helga Von Schabbs, who has sequestered herself in Castle Wolfenstein. B.J. is sent to infiltrate the castle to find Deathshead's base and learn about the dark secrets surrounding it.

The game plays almost identically to Wolfenstein: The New Order , featuring a blend of stealth action and fast-paced first-person combat. Players can switch between play styles to suit their preference. Health and armour meters are present, with 20 points of health regenerating at a time. Armour can be scavenged from corpses or found in hidden areas, also hiding treasures and goodies. Combat involves modified historical weapons that can be dual-wielded for extra power but decreased accuracy. New features include pipes, which replace knives and allow B.J. to tamper with electrical devices and climb certain walls. A simple cover system allows players to hide behind cover automatically.

Challenges are new to this game, unlocked as you progress through the campaign. These mini-challenges allow you to survive foes and obtain high scores by completing specific tasks.
