The player can choose between two protagonists, Corvo or Emily, as the main character for the entire game. Both have supernatural abilities that can be used to overcome challenges. The game offers a mix of stealth, action, and exploration, with a focus on strategy and tactics. Unlike in the first game, both characters are voiced and speak throughout the game.

The game takes place in the coastal city of Karnaca, which serves as an open world-style environment with large sandbox areas for missions. The player can explore the environment freely and complete side quests or optional activities in a random order. There are nine large missions to complete, each with distinct environments and mechanics. A ship called the Dreadful Whale is used to transport the character between missions.

Both Corvo and Emily have access to supernatural abilities known as the Outsider's Mark, which can be upgraded using runes or purchased from black markets. Abilities include distractions, deception, and combat options, with a system for tracking chaos levels that affects the outcome of the game. The player must balance aggression and stealth to succeed.

Other gameplay features include a bone charm system, which allows players to craft custom charms that provide new powers or enhance existing abilities. Blueprints can be collected to upgrade the arsenal, and paintings can be found as optional collectibles. The character's actions have consequences, influencing the game world and its inhabitants.
