Hokuto no Ken 3 is set in a post-apocalyptic world where a demonic attack has devastated the human population. Mankind needs the power of the Northern Star to rebuild and survive against demon lords and their minions. The player takes control of Kenshirou, the hero of the series, as he embarks on a quest to save humanity and defeat evil.

The game is an action & adventure RPG where players roam the wasteland, visit towns, and explore dangerous dungeons. Along the way, Kenshirou and his friends will encounter various enemies that they must defeat in turn-based battles. The battle screen features anime-style pictures of both parties, with individual one-on-one battles against bosses.
