The story takes place in 2015, where another outbreak of the Alpha-worm parasites occurs in Tokyo. The team G.U.I.D.E.-squad is sent to deal with the situation, and part of the team is Ken Ogawa, who finds himself betrayed by his own father. Driven by revenge, Ken must control him from a 3rd-person perspective through nine long levels.
Gameplay-wise, Ninja Blade plays similarly to Devil May Cry 4, with the player using three different kinds of swords and an oversized throwing star to deal with infected enemies. The weapons are upgradeable using Blood Crystals dropped by killed enemies. The throwing star also gains additional abilities during the game, such as turning into a fire extinguisher or burning projectile. There are jump-'n'-run elements included, requiring the player to use wall-walks or swinging on rails to advance through levels.
Another core gameplay mechanic is Quick Time Events, which are used throughout the levels. The game constantly switches between action and Quick Time Events, indicated by a quick zoom onto Ken's eye. Players must follow on-screen instructions to progress, with failure resulting in rewinding time back to the start of the event.
The player's performance at the end of each level is rated, granting additional Blood Crystals or unlocking new customization options for Ken.
