The fourth installment of the Sakura Taisen 4 series follows Ichiro Ogami as he returns to Japan after defeating an evil threat in Paris. Two street cars go berserk without reason, prompting the Hanagumi Brigade's mech-commanding girls to join forces with the original cast and help Ichiro defeat evil once again.

The gameplay of Sakura Taisen 4 remains largely unchanged from its predecessor, utilizing the same combat scheme that requires timing and strategy to attract the attention of the lovely girls you command. Unlike the previous games, this installment does not feature chapters, instead presenting a continuous experience.
