The story of Pia Carrot & Can Can Bunny Extra DX follows Yusuke Kinoshita, an 18-year-old male Japanese high-school student who dreams of a cool summer vacation. However, when he realizes he has no money, he takes a job at his father's restaurant, "Pia Carrot", marking the beginning of a more exciting adventure than expected.
The gameplay is focused on making schedules and choosing activities for Yusuke in the restaurant. The player chooses an action at the start of each day, such as calling one of the girls or going to different locations, before heading to work. At the restaurant, the player creates a schedule for Yusuke, sending him to various places like the kitchen or basement. After work, the player must decide which girl to talk to, with many events prompting conversation options.
The player's actions influence Yusuke's statistics, making him more or less attractive to the girls he meets. The way the player shapes and schedules activities leads to different narratives, new encounters, and eventually winning the heart of a loved one if persistence is maintained.
