The player slips into the mighty boots of Delta, making his way through the city of Rapture in a first-person-shooter-style to find and confront Dr. Sophia Lamb, who holds Eleanor against her will. The switch of perspective brings changes in gameplay, including the ability to use both plasmids and weapons at the same time, allowing for different tactics against enemies.

As with the previous game, both weapons and plasmids are upgradeable. The player can choose to adopt a Little Sister instead of harvesting it, and she will sit on his shoulder, looking for Adam-infused bodies to harvest. Setting down a Little Sister to collect bodies will require the player to protect her from splicers, and he can either harvest her himself or free her, affecting his Adam supply.

Another new feature is the hacking mini-game, which requires hitting specific areas on a gauge with a constantly spinning needle. Successful hacks lower prices at vending machines or turn turrets over to the player's side, while failed attempts result in electro-shocks or alarms. The player can also fight and walk around while hacking, as well as hack from a distance.
