Blood Omen 2 follows the story of vampire Kain, who has assembled an army to take over the world of humans. However, he is defeated by the Serafan Order and their leader, leaving his sword, the Soul Reaver, in enemy hands. Kain awakes after 200 years and seeks revenge against those who defeated him, determined to reclaim his place as ruler of the vampires.

The Legacy of Kain Series: Blood Omen 2 is a third-person action game with adventure elements. The player controls Kain from behind his shoulder, moving him in different directions and using abilities such as dark gifts and melee attacks. Kain's life energy depletes constantly, requiring him to drink blood to replenish it. As he consumes blood, his meter fills up, granting additional health.

The Serafan Order uses glyph magic, which is deadly to vampires. The player must often find ways around glyph barriers to progress through the game. Glyphs also power machines and switches, requiring the player to create circuits to deliver energy to specific points. The player can see the path of glyph energy as it travels down its path, allowing them to solve puzzles.
