The game follows Max Payne as he moves from Hoboken, New Jersey to São Paulo, Brazil in search of a new life. He works for Rodrigo Branco, but his world is turned upside down when Branco's wife Fabiana is captured by a street gang during a party. Payne sets out to rescue her, but the events quickly spiral out of control.
The game is played as a third-person shooter where Max Payne must use the series' prime bullet time mechanic to survive. Activating bullet time slows down time, allowing him to jump, dodge, shoot, and lock onto multiple enemies. The gameplay incorporates a cover mechanic, where Max can take cover near objects to avoid taking damage.
The game features various mechanics such as collecting weapons, dual-wielding pistols, and using shotguns and automatic weapons. Max can also aim and shoot from cover positions, and must constantly use cover to survive. His health is shown through an outlined figure on the right side of the screen, and he must pick up painkillers to avoid dying.
Players can examine certain objects for clues, shoot flashing buttons to open doors, and use a bullet camera to follow projectiles until impact. The game also features an arcade mode, New York Minute mode, and a multiplayer mode with a new team-based gameplay style.
The game's multiplayer mode includes regular deathmatch games, Gang Wars team mode, and variants of capture-and-hold and capture-the-flag modes. Players can also use the bullet time mechanic to affect every opponent in their line-of-sight.
