The gameplay is similar to its previous versions, but with some new features. Players can place buildings and roads with different themes, ranging from New York-style buildings to Asian-themed and European-style buildings, as well as a futuristic-type building. The game also includes disasters like meteors, fires, floods, earthquakes, and monsters that the player can use to destroy their city.

One of the main differences in this version is the way the player builds their city using the Wii Remote. Players can draw roads with the remote, making them straight or curvy as they want, and even shape neighborhoods to fit their design. The remote also allows for more destruction options, such as creating a whirlwind by spinning it vertically or smashing down on buildings like a hammer.

Assistants can be hired to help the player run the city and construct new buildings. Additionally, players can take a bird's-eye view of their city using an airplane or helicopter to assess their creation.
