Manhunt 2 takes place in the Dixmor Insane Asylum, where patients are subjected to brutal treatment. A power outage sparks chaos, and the player must navigate through the asylum and other locations as either Dr. Daniel Lamb or Leo Kasper.
The game's mechanics focus on stealth, with the addition of third-person shooter elements later in the game. Players must use cover, maneuvers, and stealth to succeed, aided by a radar system that tracks enemy awareness levels. Distracting enemies can be done through noise-making or throwing objects, while hiding requires completing a mini-game.
Players can perform executions with three styles: Hasty, Violent, and Gruesome, which feature explicit violence. However, some kills are censored in the Wii version. The game's controls on Wii use the combination of the Wiimote and Nunchuk to mimic kill motions.
All versions share similarities, but the Wii edition includes exclusive weapons like a bottle, mace, and razor. Other versions feature additional content such as a samurai sword and tutorial in the Wii version. The Wii edition also boasts more detailed environments, additional dialogue, new hunters, and enhanced animations for character interactions.
