On waking in a hospital bed, Rion is met with confusion and the presence of a mysterious voice within his mind, urging him to save a girl named Lila. As he tries to escape, he's detained by guards, revealing his extraordinary psychic abilities that have been unleashed upon him.

Galerians is an action-adventure game where players control Rion as he navigates through 3D environments, using character-specific controls reminiscent of Alone in the Dark. The objective is to collect items to unlock new areas and progress through the story. Unlike typical survival horror games, Rion cannot use weapons, instead relying on his psychic powers to fend off enemies. These abilities need to be replenished by taking certain drugs, while a special meter can trigger uncontrollable rage.
