Blood Drive is set in a post-apocalyptic world where zombies roam free. The player takes part in the Blood Drive Tournament in Las Ruletas, competing against eight hardened criminals who have managed to survive the zombie apocalypse. With weapons mounted on their cars, they must deal with both opponents and roaming zombies.

The gameplay of Blood Drive offers a variety of match types to choose from, including regular races, checkpoint races, and killing as many opponents or zombies as possible. The player can select from seven different match variations and customize their car and weapon setup before each race. During the matches, power-ups such as speed boosts and missiles are hidden throughout the track for players to find.
